* Insurance plans as indicated above, do not represent a promise or guarantee of payment, and reasonably may require a co-pay and/or deductable payment as well as any applicable exclusions and denial of non-covered services.

As a courtesy, our insurance department will call and verify in writing the medical benefits covered by your plan.

Special Note: Regardless of the medical service you are interested in utilizing, Chiropractic, Denistry or Medical, understand the difference of hearing, or reading 'Our office takes or accepts your insurance ' Verse 'We are a Contracted Provider for your insurance plan.' There is a difference. Contracted, you pay less. Non-contracted, you pay more, (non-contracted offices will say they take or accept your insurance, they won't say they are contracted, when they are not. Remember to ask the right question, 'Is your office a Contracted or Non-contracted provider of my insurance plan?'
